Diagnostic Procedures

Beginning the infertility investigation with a new patient is much like opening a mystery novel.It is most effective to begin with the simple evaluation methods and move to the more complex. Because of the myriad causes of and contributing factors to infertility, it is important to use a process of elimination, determining what the problems are not in order to better understand what they are.

There are several components that make up the basic and the advanced work up for fertility potential. Typically, after review of the health history for both partners, a basic screening comprised of hormone values and a complete pelvic exam for the female and semen analysis for the male will be obtained. If subfertile sperm parameters are discovered, referral for urological consultation will be made. Transvaginal sonograms (ultrasound examinations) are essential in the beginning evaluation for the female as well as during many treatment modalities. Saline enhanced sonograms (sonohystograms) are commonly used to evaluate the uterine cavity and rule out abnormal masses that could interfere with conception. It is critically important to determine if the fallopian tubes are open and this is commonly accomplished by a simple, outpatient procedure in the Radiology Department of a hospital. This procedure is called a hystosalpingogram.

In some patients, especially those who have pelvic pain or recurrent pregnancy loss, a preferable method of evaluating the uterus and tubes is with outpatient laparoscopy and hysteroscopy. Laser technology allows the surgeon to rid the patient’s pelvis of scar tissue, adhesions and endometriosis during the same procedure that provides him with information about the uterus, tubes and ovaries.

Although there are common approaches to the evaluation of infertility, each woman and her partner are unique in their specific situation and deserve individualized care. We believe that our patients are in partnership with us to seek the most thorough explanation of “why” successful pregnancy has not occurred and to formulate a plan that will have the greatest potential to help them in their quest for a child.
